To have this filed by TTI, fill out the form below and proceed to our shopping page where you will select the applicable quantity and service and complete the payment process. Once payment is received, a filings specialist will contact you within 24 hours to file on your behalf during regular office hours of Monday – Friday, 8:00 AM – 5:00 PM CST. Our BOC-3 filing fee includes: BOC-3 filing, BOC-3 form copy (mailed or faxed to you within 24 business hours), Tracking and Notification of Authority Publication, and a copy of Certificate of Authority.
*** Our BOC-3 forms are permanent. However, if you request a name change for your authority your BOC-3 form will lapse from your authority and will need to be re-filed. We charge a $35.00 name change fee to all previous clients. All individuals not previously holding a BOC-3 form with us must pay original BOC-3 filing prices.****
